Conference Cost

Fees for the conference are outlined below. Colleagues wishing to attend from NHS England or from the Health Protection Agency are exempt from paying Value Added Tax (VAT). The fees including VAT are shown in the right hand column. A reduction for early registration is available for those registering before 30th March 2005.

Payment methods

You will be required to select a payment method option when you register:

Invoice - To be invoiced and pay within 14 days of receipt by cheque or bank transfer you will need to provide a purchase order number and address to which the invoice should be sent.

Cheque - To pay by cheque now, cheques should be made payable to "Health Protection Agency" (c/o DBH House, 105 Boundary St, Liverpool, L5 9YJ). VAT should be included for delegates other than Health Protection Agency and NHS employees in England. Instructions for sending payment are on the reservation confirmation screen.

Internal recharge - For employees of the Health Protection Agency an internal recharge is possible if you provide your department cost code. Please ensure you have your manager’s permission to attend the conference before registering.

Your place is only confirmed once payment has been received.

Amendments to registration

If you need to change other details your reservation please email zoonosesconference@hpa.org.uk with the amendments and your booking reference number.

Closing date

Registrations may be accepted up until 03 June 2005, subject to availability.
Payments must be received by 03 June 2005.

Cancellation policy

A 25% cancellation fee will be charged on cancellations sent by email to zoonosesconference@hpa.org.uk before 27 May 2005. Refunds will not be possible after 27 May 2005; however, a substitute delegate will be possible at any time.

Continuing Professional Development (CPD) Accreditation

Accreditation of the conference for CPD purposes has been obtained from the Faculty of Public Health. 15 credits are available for the whole conference on return of an evaluation form. 6 credits are available for Wednesday and Thursday and 3 credits for Friday.
